We are recruiting for an experienced Adult Nurse Case Manager / Clinical Lead to join a specialist complex care provider supporting adults with long-term conditions in their own homes across London and the South East. In this role, you will:
- Manage a caseload of adult complex care clients across London & surrounding areas
- Conduct assessments and create personalised care plans
- Deliver face-to-face clinical support to clients and their families
- Train and supervise carers to achieve clinical competency
- Work collaboratively with MDT teams and local NHS partners
- Ensure all care is safe, compliant, and reflective of individual needs
