We’re recruiting for an experienced Care Coordinator to join a specialist Complex Care provider delivering high-quality, person-centred care in clients’ own homes. This is a dynamic role where you’ll manage complex care packages, coordinate care teams, and ensure services are delivered safely and effectively.

In this role, you will:
- Manage a caseload of complex care packages and oversee day-to-day operations
- Recruit, interview, and induct Healthcare Assistants and Support Workers
- Build strong care teams and ensure contingency staffing plans are in place
- Manage rotas, supervisions, spot checks, and staff appraisals
- Attend MDT meetings and liaise with healthcare professionals and commissioners
- Conduct care plan reviews alongside clinical colleagues
- Ensure staff training and clinical competencies remain up to date
- Liaise regularly with clients and families to review care delivery
- Investigate concerns or complaints and escalate appropriately
- Maintain accurate records and reports to support quality service delivery
- Undertake on call responsibilities on a rota basis
