We are seeking a compassionate and detail-oriented Care Co-ordinator to join our dedicated team. This vital role involves overseeing and coordinating the rota, as well as developing comprehensive care plans for individuals with diverse needs, ensuring they receive personalized, high-quality support in various settings including assisted living, home care, and community environments.

The care coordination is covering a wide area across several localities, with client with a variety of care needs. The ideal candidate will be a driver with their own vehicle, possess strong organizational skills, a deep understanding of patient care, and experience working with individuals with disabilities, dementia, or developmental conditions. This position offers an opportunity to make a meaningful difference in clients’ lives by coordinating exceptional caregiving and support services.

Responsibilities:

• .Organise staff rotas, develop, implement, and monitor individualized care plans tailored to each resident’s or patient’s needs.
• .Coordinate with healthcare professionals, families, and support staff to ensure seamless delivery of care.
• .Carry out spot checks and staff supervisions for care team members
• .Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance.
• .Administer and order medications and monitor vital signs
• Provide specialized care such as Alzheimer’s/dementia care and memory care,
• .Support service users with meal preparation, nutritional needs, and feeding assistance while ensuring safety protocols are followed.
• .Conduct patient observation to identify changes in condition or behaviour requiring attention or intervention.
• .Operate mobility aids safely and assist with heavy lifting when necessary.
• .Maintain accurate documentation of care activities, medication administration, and patient progress in compliance with healthcare standards.
• .Ensure a clean and safe environment by assisting with light cleaning duties as needed.
• .Educate families and support staff on care procedures
• . Assist with covering shift where and when necessary.

Experience:

• Prior experience in home health care, or care environments is highly preferred.
• Demonstrated knowledge of patient observation techniques, medication administration, vital signs monitoring, and dementia or Alzheimer’s care.
• Experience working with individuals with developmental disabilities or autism is advantageous.
• Familiarity with HIPAA regulations and maintaining patient confidentiality is essential.
• Hands-on experience in long-term care or home & community care settings is desirable.
• Certifications such as First Aid, CPR, or Nursing Assistant credentials are beneficial but not mandatory; training can be provided if necessary.
• Strong interpersonal skills to work effectively with residents, families, healthcare teams, and support staff.
