This is an exciting post within Digital Health, designed to support the Virtual Ward Team.
The successful candidate will work within the Digital Health Team supporting patients to remain at home using remote monitoring and video assessments. These patients are supported from GP Ward round and a Consultant ward round daily which you will provide support to. The Virtual ward service is a busy and developing service; if you are someone looking for your next challenge and like change, then we would love to hear from you.
Main duties of the job:- The RNA will work as part of the Digital Health Team, be multi-skilled and will work across various disciplines within Digital Health and organisations including SFT.
- They will provide planned care and be able to take appropriate action to achieve the planned aims.
- The RNA will be proficient, competent and require minimum supervision.
The role requires the RNA technical, clinical skills of the post will be further enhanced as part of an induction.

Qualifications:
- Essential: Foundation degree in Health and Social Care, or RNA Qualifications
Experience:
- Essential: Experience of working within a healthcare environment
- Awareness of development needs to meet role
Skills and abilities:
- Can maintain accurate patient / nursing records
- Demonstrate effective verbal and written communication
- Provide care for patients following a designated care plan
- IT / Computer literacy
- Willingness to undertake the training required to carry out the job
- Demonstrate an awareness of patient safety including safeguarding procedures
- Willingness to abide by the Trust policies and procedures
