Job Title

Clinical Lead Speech and Language Therapy for CNRT – 29 hours per week.

Job Summary

Exciting opportunity to be the Clinical Lead for Speech and Language therapy within CNRT. You will devise, coordinate and implement service delivery, work within a multidisciplinary team and be active in the development and promotion of integrated neuro‑rehabilitation services in the community. You will act as a specialist Speech and Language therapist within the field of neurological rehabilitation and will offer specialist assessments, treatment and management plans to patients with complex neurological conditions. You will also provide expert guidance and education to patients, families, carers and the wider multidisciplinary team.

Main Duties

  • Provide an advanced specialist service for CNRT, a specialist neuro‑rehabilitation team for adults with acquired neurological disorders, in a community setting.
  • Work as a member of a team, providing specialist assessment, diagnosis and treatment for patients with varied and complex communication and swallowing difficulties.
  • Carry a personal caseload of adult clients and deliver a Specialist SLT service as part of an integrated multi‑disciplinary team.
  • Provide supervision and advice in own specialist area to other therapists, assistants and students.
  • Maintain professional specialist knowledge and skills in adult neurology acquired through postgraduate SLT training, experience and increased depth and breadth of clinical competence.

Responsibilities

  • Manage a specialist caseload of adult clients, including patients with communication, swallowing and voice difficulties.
  • Carry full responsibility for assessment, diagnosis, treatment and discharge.
  • Assess and diagnose all in and out patients in both general and own specialist caseloads.
  • Work collaboratively with clients, families, carers and other professionals during assessment and all subsequent management, including decision making.
  • Communicate complex condition‑related information from assessment to clients and their families/carers, including difficult or unwelcome information about seriousness or long‑term nature of conditions.
  • Provide specialist advice to patients, carers, nursing staff, allied health professionals, medical staff and others regarding the management/treatment of patients with communication and/or swallowing difficulties.
  • Work in conjunction with team leaders and demonstrate skills to identify and deliver care plans for highly complex cases, generating and implementing appropriate strategies for caseload management.
  • Provide written reports, presenting complex information in a clear and accessible way for carers, GPs, medical consultants and other allied health professionals.
  • Direct clients/carers to appropriate voluntary agencies and external services for support and information, e.g. The Stroke Association and other charitable groups.
  • Participate in the delivery and development of specialist training in many settings (e.g. Nursing Home, CNRT department) to nursing staff, AHPs, carers. Training will cover communication, dysphagia or both.
  • Take a lead role in developing and delivering specialist training (formal and informal) to others in the area of clinical expertise.
  • Work with technical instructors/Speech and Language Therapy Assistants, providing programmes of therapy to be used with individual patients and supervising their implementation.
  • Provide training for staff within the department and supervise students and junior specialists in own specialist areas, ensuring teaching meets the standards of the degree level qualification.
  • Act as mentor for specialist SLT and junior staff, providing advice and support to assistants and volunteers.
  • Maintain comprehensive, accurate and up‑to‑date case notes for own clients, adhering to professional and trust standards.
  • Undertake audit and data collection as required by the trust and submit timely and accurate data, including patient contacts, caseload figures and activity levels.
  • Be responsible for line management of lower banding staff.
  • Be responsible for developing and monitoring clinical guidelines and patient pathways in your specialist area.
  • Embrace and support line management and the department in making service improvements effective within the workplace.
  • Develop care protocols/packages relating to specialist area in liaison with Team Leader to improve patient care.
  • Contribute to interagency/multi‑disciplinary team building and policy development.
  • Travel between diverse clinical and non‑clinical environments and bases (patients\’ homes, GP clinics, primary care facilities, hospitals, office spaces and residential/nursing homes) as needed. Reliable transport is essential. The post holder will need to complete an agreed number of clinical contacts a week at separate addresses to maintain provision of services when working within CNRT.

Person Specification

Education and Qualification

  • Essential: Degree or equivalent in SALT; HCPC registration; Evidence of postgraduate education in clinical area.
  • Desirable: Management and leadership training.

Knowledge

  • Essential: Knowledge of relevant conditions.

Experience

  • Essential: Working within an MDT; Developing and implementing training programmes.
  • Desirable: Working in the community.
