What you’ll do:

  • Person-centered planning: Chat with the client (and family/advocates), get to know them and understanding of goals and routines.
  • Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan.
  • Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
  • Social time and participation: Bring meaning, fun, and community access aligned with interests.
  • Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
  • Cultural humility: Respect backgrounds, beliefs, and ways of being.
  • Showering/Dressing assistance
  • Light Domestic duties
  • Medication prompting
  • Assistance with food

Who we’re looking for:

What are the shifts:

Mondays 10am to 1pm and 1:30pm to 4:30pm and alternative weekends.
