What you’ll do:
- Person-centered planning: Chat with the client (and family/advocates), get to know them and understanding of goals and routines.
- Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan.
- Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
- Social time and participation: Bring meaning, fun, and community access aligned with interests.
- Autonomy and advocacy: Support decision-making, respect consent, boost independence where possible.
- Cultural humility: Respect backgrounds, beliefs, and ways of being.
