Clapham Park Group Practice are looking for a friendly and enthusiastic GP to join our growing multidisciplinary team.

• Work with a multidisciplinary team of 1 Advanced Nurse Practitioner, 3 Pharmacists, 1 Pharmacy Technician, 1 Social Prescriber and large nursing and administrative teams.

• 6 weeks annual leave plus 10 statutory days and bank holidays.

Main duties of the job

• Provide high quality care to our patients
• Conduct face-to-face, telephone and online consultations
• Form part of our total triage team on a pro‑rate basis with the other GPs
• Undertake home visits
• Manage acute and chronic conditions in line with national and local guidelines
• Issue prescriptions appropriately in accordance with practice and NHS policy
• Review test results, hospital letters and correspondence promptly
• Ensure accurate and timely clinical record keeping using EMIS WEB
• Support the training and development of GP registrars, F2s and Allied Health Professionals through clinical supervision

Job Description

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.

• Clinical responsibilities
• Make yourself available to undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
• Make professional, autonomous decisions in relation to presenting problems, whether self‑referred or referred from other health care workers within the organisation.
• Assess the health care needs of patients with undifferentiated and undiagnosed problems.
• Screen patients for disease risk factors and early signs of illness.
• Develop care plans for health in consultation with patients and in line with current practice disease management protocols.
• Provide counselling and health education.
• Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
• Record clear and contemporaneous consultation notes to agreed standards.
• Compile and issue computer‑generated acute and repeat prescriptions (avoiding hand‑written prescriptions whenever possible).
• Prescribe in accordance with the practice prescribing formulary (or generically) whenever clinically appropriate.
• Undertake all normal duties and responsibilities associated with a GP working within primary care.

• Awareness of and compliance with all relevant practice policies/guidelines, e. g. prescribing, confidentiality, data protection, health and safety.
• A commitment to life‑long learning and audit to ensure evidence‑based best practice.

• Contribute to the development of computer‑based patient records.
• Contribute to the summarising of patient records and read‑coding patient data.

• Manage infection control procedures in routine and extraordinary circumstances, including pandemic situations.
• Maintain hand hygiene standards for self and others.
• Manage incidents of accidental exposure.
• Implement infection control protocols across the practice.
• Observe current working practices and identify training needs, escalating issues as appropriate.
• Manage risks involved in work activities in a way that protects patients and staff.
• Use training to update knowledge and skills and manage staff training across infection control and patient processes.
• Monitor practice facilities and equipment for infection control, ensuring adequate hand cleansing facilities.
• Safely manage sharps use, storage and disposal.
• Maintain a clean working environment.
• Initiate remedial or corrective action where needed.
• Always work in a tidy, clean, sterile, and safe way, reporting hazards promptly.
• Maintain general cleanliness across the business in consultation with responsible managers.
• Participate in periodic infection control training (minimum twice annually).

Qualifications

• GMC Registered, not subject to suspension.
• Currently on a CCG performers list.
• Clear enhanced DBS check.
• Evidence of CPD.
• MRCGP.
• DRCOG.
• DFSRFH and competence in coil insertion.
• Work as a GPWSI.
• Minor surgery skills.

Experience

• Experience of working in UK General Practice.
• Experience of working with vulnerable groups.
• Teaching of registrars or F2 or medical students.
• Delivery of clinical audit.
• QOF management.
• Experience using EMIS Web and Docman.
