Are you an experienced RGN ready to step into a leadership role where you can genuinely shape care delivery? At Claimont Health, we’re looking for a Nurse Team Lead to support and oversee complex care packages across Sussex and West Sussex. This is a hands-on leadership role. You won’t just be managing from a distance, you’ll be actively involved in care delivery, supporting your team on the ground and ensuring the highest standards of clinical practice.
About the Role
You’ll be supporting adults with complex clinical needs within their own homes, which may include neurological conditions, long-term health conditions, rehabilitation support and other complex physical health needs. This role requires a high level of clinical awareness, risk management confidence and professionalism in delivering care in the community. You will work closely with our Clinical Lead and wider operational team to ensure safe, effective and personalised care delivery. Alongside your clinical responsibilities, you will lead and support a team of support workers, ensuring consistency, quality and a strong person-centred approach across all packages.
Key Responsibilities
- Assess and monitor clients’ health and wellbeing
- Develop, implement and review person-centred care plans
- Provide clinical leadership and oversight across care packages
- Support, mentor and guide Support Workers in the field
- Participate in and cover shifts where required, ensuring continuity of care
- Administer medication and monitor for effectiveness and adverse reactions
- Maintain accurate and timely clinical documentation
- Promote safeguarding, safety and best practice at all times
- Work collaboratively with families, clients and multidisciplinary teams
- Support delivery of complex interventions and delegated clinical tasks
What We’re Looking For
- Registered General Nurse (RGN) with valid NMC registration
- Strong experience within complex care and community-based nursing
- Previous lead or senior experience supporting teams
- Confident working autonomously and making clinical decisions
- Full UK driving licence and access to a car (essential)
- Willingness and flexibility to travel across Sussex / West Sussex
- Ability to support shifts when required
- Calm, professional and compassionate approach
