About the Role
You’ll be supporting adults with complex and enduring mental health needs, which may include trauma-related presentations, personality disorders, psychosis, mood disorders and co-existing physical health conditions. This role requires a high level of clinical awareness, risk management confidence and professionalism in delivering care. You will work closely with our Clinical Lead and wider operational team to ensure safe, effective and personalised care delivery in the community.
Duties
- Assess and monitor patients’ mental health status, ensuring their safety and well-being at all times.
- Develop, implement, and evaluate personalised care plans in collaboration with patients and multidisciplinary teams.
- Administer prescribed medications accurately and observe their effects, reporting any adverse reactions.
- Provide emotional support and counselling to patients experiencing mental health challenges.
- Document patient progress, treatment outcomes, and any incidents in accordance with organisational policies.
- Promote a safe environment by adhering to safeguarding procedures and infection control protocols.
- Educate patients and their families about mental health conditions, treatment options, and coping strategies.
- Participate in team meetings, training sessions, and continuous professional development activities.
