Claimont Health is recruiting a fantastic role as a Registered General Nurse based in the community to support our client who has complex health needs and requires a high level of care and support at home. You will be supporting our Client with Dialysis.

As a specialist in your field, you will undertake all aspects of clinical duties as an autonomous practitioner working within your professional scope of practice. Main duties of the job

  • Providing complex care to clients with needs of varying complexity in the community under the guidance of the Registered Manager.
  • Supporting our Client with Dialysis.
  • Devising treatment programmes to address the recovery needs and physical wellbeing of individuals with varying mental health needs.
  • Undertaking mental health assessments and formulating care plans in conjunction with the MDT, patient, and their family/carers.
  • Carrying out thorough risk assessments and creating risk management plans.
  • Identifying any specific precautions or contraindications to the proposed interventions/treatments and taking the appropriate action.
  • Undertaking and recording interventions/treatments correctly, and in line with legislation, policies, and procedures and/or established protocols.
  • Ensuring that written records of all aspects of service user treatment and progress are maintained throughout the treatment process.
  • Working with families/carers to assist them in understanding mental health and coping effectively with issues arising from their role as carers.
  • Reporting assessment findings in the appropriate format to the people who need them.
  • Ensuring confidentiality and security of patient data is always maintained in compliance with the Data Protection Act.
  • Making clinical decisions based on specialist knowledge of the client's individual needs and care plan protocols and responding to any change with the client's condition.

Job responsibilities

Claimont Health is recruiting a fantastic full-time role as a Registered General Nurse based in the community to support clients who have complex clinical needs and require a high level of care and support at home. The post holder will benefit from the support and motivation of both the clinical and operations teams and will provide support to long-term conditions and frailty clients in the comforts of their own homes. The post holder will undertake all aspects of clinical duties and will work within their professional scope of practice to deliver individualised care that supports patients to achieve a desired outcome. They will be responsible for the co-ordination and the delivery of high-quality mental health care using evidence-based practice through the provision of clinical advice and support, acting as a role model, and providing education and training where appropriate. They will liaise closely with external professionals, family and carers, local services, and anyone else involved in the patient's care package. The job requires a car driver and travel, as and when required across England.

Your experience will include but is not limited to:

  • At least 3 years of experience as a Nurse.
  • Experience of Dialysis.
  • Demonstrate understanding of Clinical Governance; experience of RADAR or a similar risk management system will be beneficial.
  • Experience of maintaining effective clinical risk register and developing plans to mitigate against identified risks.
  • Experience and knowledge of quality issues and outcome measures and how to implement them effectively.
  • Good understanding of multidisciplinary working and interprofessional relationships.
  • Understanding of relevant legislations and research awareness.
  • Have knowledge of up-to-date clinical practice and current issues relating to the profession.

You will be able to:

  • Plan, prioritise and manage own caseloads.
  • Have a problem-solving approach including data interpretation and scheduling.
  • Demonstrate excellent communication and organisational skills, with a solution-driven, dynamic approach to prioritising your workload.
  • High level of attention to detail, accuracy and pride in your work presentation.
  • Strong written and verbal communication skills and the ability to multi-task.
  • Be able to work on your own initiative and/or as part of a team across multiple areas.
  • Willingness to undertake additional responsibilities to support the organisation as and when required.

Person Specification

  • Qualifications: Degree in professional nursing, Professional Registration, Evidence of continued professional development appropriate for the role, Evidence of post-graduate education in specialist clinical area.
  • Experience: Experience working with people with Renal issues, Experience of working in a community setting, Experience of working within a Nursing team, Experience of dealing with vulnerable patients, Experience of managing a team.
