Job Purpose
Clackmannanshire Council is promoting Community Connecting as a way of reducing or mitigating inequalities. Community Connecting links people seeking help from public services with local community resources to find services and activities that offer practical, social and emotional support to improve people’s lives. The programme brings together Clackmannanshire Council, CTSI and the wider voluntary and community sector to work in partnership to support local adults. This role links specifically with Strive a multi agency support team, alongside the Alcohol and Drug Partnership and Housing Services, working with people who have been identified by staff as likely to benefit from a Community Connecting approach that will positively improve their outcomes.
Approach
You’ll usually work with someone over several sessions, depending on their needs, though it is important that you maintain clear boundaries and do not create a dependence. Your expertise and experience will enable you to actively listen to their issues and concerns, and have good quality conversations to explore what might be required to meet their needs. Your holistic approach and excellent interpersonal skills will mean that you provide support and encouragement to people around a wide range of aspects of their wellbeing. You will provide them with information and support around access to groups and activities to help build their confidence and wellbeing. This support will enable them to break down social isolation, loneliness and bring lasting improvements to their lives.
Duties and Responsibilities
- Offer a specialist, professional service to people experiencing complex social circumstances through meaningful conversations.
- Build trusting, non-dependent relationships to enable people to identify personal outcomes and priorities for improving their mental and physical health and well-being.
- Support people in overcoming barriers and, where necessary, connect them with relevant local services or activities.
- Meet and connect with them in places where they feel comfortable (this can include but is not limited to community venues, wellbeing walks, home visits etc)
- Assist people referred by practitioners by providing guidance and signposting, to facilitate access to community resources. Empower people to take control of their wellbeing through tailored support.
- Manage a caseload of people providing ad-hoc support as required.
- Establish and maintain efficient pathways for accessing Community Connecting, ensuring minimal barriers to engagement and timely 1:1 appointments.
- Cultivate excellent working relationships with statutory, public, and third-sector service providers to simplify complex pathways for people.
- Share knowledge of local and national resources with public sector staff and colleagues, enhancing service coordination.
- Support public sector staff to adopt community-oriented approaches to people’s wellbeing.
- Maintain accurate records for each person using the CTSI MILO management system including recording a core dataset to enable effective monitoring and evaluation of the programme.
- Collaborate with evaluation colleagues to ensure data informs service improvement and deliver outcome reporting in a timely manner to aid learning and development.
- Ensure compliance with data-sharing protocols and monitor service outcomes to meet CTSI reporting requirements for the funder, Clackmannanshire Council.
- Work collaboratively to identify and address gaps in community services. When appropriate, partner with stakeholders to create new initiatives and engage actively in CTSI and partner meetings to support service development.
- Develop strong connections with local community groups and identify key support networks, such as befriending services, physical activities, and social opportunities. Maintain up-to-date knowledge of available services enabling greater visibility and accessibility. Raise awareness of service gaps and advocate for areas of need within the community.
- Promote volunteering as a means of personal improvement and as a tool for achieving better outcomes. Collaborate with volunteer Community Health Champions/Connectors to extend the service's reach, particularly to individuals facing significant barriers. Work with CTSI colleagues to recruit, train, and support these volunteers.
