ROLE AND RESPONSIBILITIES

We are looking to employ an in-house Rehabilitation Co-ordinator based out of our Stoke office.

The role will involve working alongside our clients and their families, in cases involving varied complex medical/mental health, cognitive and physical needs arising from major trauma.

The role will typically involve assisting with the transition from hospital to home, assisting in the identification of a suitable Case Manager, helping to reduce the stress and challenges involved after a serious injury and working alongside the injured person and their family, the treating medical professionals, and additional support where required.

You will ideally have experience in connection with disability teams and /or community review teams and /or multi-disciplinary teams, and the confidence and experience to liaise effectively with Case Managers, Rehabilitation Consultants, Occupation Therapists, Physiotherapists and other treating clinicians, social services, housing departments, local authorities, education authorities and charities as needed to co-ordinate the injured clients rehabilitation journey.
