Social Care Facilitator to join our dynamic Adult Social Care team for a 3-month contract.
This role is pivotal in supporting vulnerable adults and their carers by assessing needs, aiding in care planning, and promoting independence.
As part of a multi-disciplinary team, you will ensure that individuals can access the necessary services and community resources to achieve positive outcomes.
This position is ideal for someone with a background in Adult Social Care, Care Management, Support Planning, or Community-Based Services.
Responsibilities
- Support the assessment of vulnerable adults and their carers.
- Contribute to support plans that promote independence, choice, and wellbeing.
- Assist service users in accessing community resources and support services.
- Undertake allocated care management duties.
- Work in a person-centred way with service users, families, and carers.
- Support individuals to maintain choice and control while considering mental capacity.
- Collaborate with social workers, healthcare professionals, and partner agencies.
- Identify the need for assistive equipment and arrange provision where appropriate.
- Refer individuals for specialist assessments where complex equipment or adaptations are required.
- Maintain accurate records and update case management systems.
