3 months contract with local authority. We are seeking an experienced agency social worker to join our Learning Disability Team, primarily serving adults with learning disabilities in the West of the borough, focusing on the Rhondda area. This role is integral to safeguarding and enhancing the well-being of adults through high-quality social work practices. You will be part of a supportive and experienced team, contributing to the assessment, care planning, and review processes.

Responsibilities:

  • Safeguard and promote the well-being of adults through high-quality social work practices.
  • Conduct assessments in accordance with the Social Services and Well-Being Act (Wales) 2014.
  • Develop, monitor and review care and support plans as needed.
  • Perform Mental Capacity Assessments and adhere to the principles of the Mental Capacity Act 2005.
  • Assess risks in a manner that encourages co-production, independence, and choice.
  • Identify and promote individual strengths.
  • Collaborate with other agencies and adopt a multi-disciplinary approach when appropriate.
  • Maintain and update electronic records, including case recordings, assessments, care and support plans, in line with departmental policies.
  • Travel independently throughout the service area, residing within a reasonable distance to fulfil this requirement.
  • Participate in remote work with one designated office day per week.

Requirements:

  • Proven experience as a social worker, preferably within a learning disability context.
  • Familiarity with the Social Services and Well-Being Act (Wales) 2014 and the Mental Capacity Act 2005.
  • Strong assessment and care planning skills.
  • Ability to work collaboratively with multi-disciplinary teams.
  • Excellent communication and organisational skills.
  • Ability to travel independently within the service area.
  • Reside within a reasonable distance to the service area.
