- Summary
- The Personal Advisor will provide high-quality support, guidance, and practical assistance to care leavers and young people transitioning into adulthood.
Working within the Leaving Care Team, the postholder will manage a diverse and complex caseload, including former and current asylum-seeking young people, individuals with unresolved immigration status, and young people with disabilities.
Responsibilities
- Manage a complex and diverse caseload of care leavers and young people aged 16â
25.
- Establish positive and trusting relationships with young people, supporting them to identify and achieve personal goals.
- Undertake comprehensive needs assessments and develop outcome-focused support plans that meet individual needs.
- Prepare, review, and maintain Pathway Plans in accordance with statutory guidance, ensuring plans are SMART, person-centred, and focused on independence.
- Support young people in areas including accommodation, education, employment, training, health, finances, immigration matters, and emotional wellbeing.
- Work in partnership with social workers, housing providers, education services, health professionals, voluntary organisations, and other stakeholders to deliver coordinated support.
Essential Experience Required
- Experience of working directly with vulnerable young people, care leavers, children in care, or similar client groups.
- Experience managing a caseload and delivering person-centred support plans.
- Experience completing assessments, reviews, reports, and detailed case recordings.
- Proven ability to work with young people presenting complex needs, including those with mental health issues.
Essential Qualifications Required
- Relevant qualification in Youth Work, Social Care, Health & Social Care, Education, Community Work, or equivalent experience.
- Enhanced DBS Check with Adults and Childrenâ s Barred List.
- Full UK Driving Licence and access to a vehicle (or ability to meet travel requirements of the role).
- Additional Information
- Working hours: 37 hours per week.
- Working arrangements: Mon to Thur 8:45 â
- Length of Assignment: 12 weeks with possible extension.
- Additional Requirements: Enhanced DBS with Adults & Children's Barred List / Driver (Driving license required).
- We work on a bi-weekly schedule.
