Join a dynamic Leaving Care Service in Lincolnshire that is expanding its team to better support Care Experienced Young People aged 16β21. This role offers a meaningful opportunity to assist young individuals in their transition to independent adulthood. As a Personal Adviser, you will provide practical guidance, emotional support, and advocacy, empowering young people with the tools and confidence needed to succeed.
Responsibilities
- Build strong, positive relationships with young people and their support networks.
- Develop and review Pathway Plans using Signs of Safety and Valuing Care frameworks.
- Support young people in key areas such as:
- Accommodation
- Health and wellbeing
- Education, employment, and training
- Finances and life skills
- Coordinate multi-agency support across housing, health, and partner services.
- Conduct statutory face-to-face visits (local and national travel required).
- Act as Lead Professional where appropriate.
- Participate in a duty rota and provide support during crisis situations.
Requirements
- Full UK driving licence and access to a vehicle (national travel required).
- Experience within a local authority or council setting is highly desirable.
- Experience supporting young people aged 16β25.
- Background in leaving care, youth work, social care, or a similar setting.
- Strong understanding of safeguarding, risk management, and vulnerability.
- Experience working within multi-agency environments.
- Proven ability to build effective, supportive relationships.
Qualifications
- NVQ Level 3 (or equivalent) in Health & Social Care, Youth Work, or a related discipline (desirable).
