Responsibilities
- Provide ongoing support to care leavers, serving as a consistent and trusted point of contact.
- Develop, implement, and review Pathway Plans in accordance with statutory requirements.
- Assist young people in developing life skills, including budgeting, cooking, and tenancy management.
- Encourage and empower young people to make informed decisions about their future.
- Help care leavers secure suitable accommodation and sustain tenancies.
- Support access to education, training, and employment opportunities.
- Offer guidance on financial management, benefits, and entitlements.
Requirements
Essential Experience Required
- Experience working with young people, particularly those in care or leaving care, is highly desirable.
- Experience in a support, youth work, housing, or social care environment.
- Experience working with vulnerable individuals and managing challenging behaviours.
Essential Qualifications Required
- NVQ Level 3 (or equivalent) in Health & Social Care, Youth Work, or a related field (desirable).
- GCSEs (or equivalent) including English and Maths.
- Willingness to undertake relevant training and development.
