Are you highly organised, detail-oriented, and experienced in care coordination or mobilisation? We are looking for a Care Package Mobilisation Coordinator to join our clients team in London and support the seamless onboarding of new complex care packages.

You will play a key role in coordinating the logistical, clinical, and administrative steps required to safely and efficiently mobilise care for clients with complex needs. This is a fantastic opportunity to make a real impact at a critical stage of the care journey.

Key Responsibilities

  • Coordinate all aspects of new care package mobilisation under the guidance of senior leadership.
  • Liaise with clients, families, commissioners, and internal teams to gather essential information.
  • Schedule initial client assessments with the Clinical Team.
  • Work with Recruitment and Coordination teams to allocate suitable care staff.
  • Prepare staff profiles and arrange introductions/meet-and-greets.
  • Create and finalise the initial rota; obtain sign-off from clients or families.
  • Maintain accurate mobilisation records using care management systems.
  • Ensure compliance with company policies and CQC regulations.
  • Provide regular updates to internal stakeholders; escalate issues where needed.
  • Handover packages to the long-term care team once mobilisation is complete.

  • Strong time management and attention to detail
  • Proficient in Microsoft Office and rostering systems
  • NVQ Level 2 or higher in Health & Social Care (or equivalent)
