Cityworx are recruiting for an experienced Care Coordinator to join an established complex care business based in the Southwark, London area. As a Care Coordinator your role is to coordinate and oversee high-quality, person-centred care for individuals with complex needs. Acting as the central point of contact between clients, families, and multidisciplinary teams, the Care Coordinator ensures that care is delivered seamlessly, compassionately, and in line with best practice. Key Responsibilities
- Act as the main point of contact for clients and families, offering clear guidance and support throughout their care journey.
- Coordinate appointments, referrals, and follow-up visits, ensuring continuity and consistency in care delivery.
- Collaborate with healthcare professionals to develop personalised care plans tailored to each client's needs.
- Monitor ongoing care, outcomes, and adjust plans as needed to achieve the best results.
- Maintain accurate records and ensure compliance with internal protocols and regulatory standards.
- Proactively manage care package mobilisation and onboarding of new clients in a timely and organised manner.
- Promote clear communication between internal teams, external partners, and families to ensure clarity of care plans and expectations.
- Take part in on-call responsibilities.
- Liaise with other departments within the company to support safe and sustainable growth.
- Manage a caseload of 10-15 complex care packages, ensuring all care delivery and compliance standards are met.
Requirements:
- 1 year experience as a Care Coordinator.
- Willing to participate in the on-call rota.
- Experience of managing complex care packages preferred.
- Participating and attending regular meetings.
- Participating in the recruitment of support workers.
- Managing and reviewing performance of complex care support workers through probation periods, management practices, supervision sessions, spot checks, training records and an annual appraisal.
