We are recruiting for an experienced Clinical Case Manager to join a specialist community-based complex care service in Exeter.
This is an excellent opportunity for a Registered Nurse with strong community complex care experience who can provide clinical leadership, support care packages and work closely with clients, families, staff and external professionals.
Key Responsibilities
- Assess clients' physical and mental health needs.
- Develop personalised, outcome-focused care plans.
- Complete clinical and environmental risk assessments.
- Review care packages in response to changing needs.
- Provide clinical support to clients and their families.
- Act as a key communication link between families, professionals and care teams.
- Support the identification and induction of staff into complex care packages.
- Provide ongoing clinical support to nurses and care staff.
- Complete staff supervisions, appraisals and competencies.
- Support training and development of clinical and non-clinical staff.
- Interview healthcare staff.
- Work with commissioners to support new care packages.
- Help reduce unplanned hospital admissions through effective case management.
- Ensure care delivery meets CQC and safeguarding requirements.
- Maintain accurate clinical records and electronic care management systems.
- Support Care Coordinators to ensure continuity of care.
- Deal with complaints and concerns appropriately.
- Participate in an out-of-hours on-call rota.
About You
- Registered Nurse with current NMC PIN registration.
- Minimum 2 years' post-qualification experience.
- Minimum 5 years' experience within health and social care.
- Experience within community-based complex care.
- Experience managing and supervising nurses or care teams.
- Strong knowledge of health and social care legislation and CQC requirements.
- Excellent communication and relationship-building skills.
- Highly organised with strong clinical leadership skills.
- Full driving licence and access to your own vehicle.
- Confident working with clients, families and multidisciplinary professionals
