We are recruiting for an experienced Clinical Case Manager to join a specialist community-based complex care service in Exeter.

This is an excellent opportunity for a Registered Nurse with strong community complex care experience who can provide clinical leadership, support care packages and work closely with clients, families, staff and external professionals.

Key Responsibilities

  • Assess clients' physical and mental health needs.
  • Develop personalised, outcome-focused care plans.
  • Complete clinical and environmental risk assessments.
  • Review care packages in response to changing needs.
  • Provide clinical support to clients and their families.
  • Act as a key communication link between families, professionals and care teams.
  • Support the identification and induction of staff into complex care packages.
  • Provide ongoing clinical support to nurses and care staff.
  • Complete staff supervisions, appraisals and competencies.
  • Support training and development of clinical and non-clinical staff.
  • Interview healthcare staff.
  • Work with commissioners to support new care packages.
  • Help reduce unplanned hospital admissions through effective case management.
  • Ensure care delivery meets CQC and safeguarding requirements.
  • Maintain accurate clinical records and electronic care management systems.
  • Support Care Coordinators to ensure continuity of care.
  • Deal with complaints and concerns appropriately.
  • Participate in an out-of-hours on-call rota.

About You

  • Registered Nurse with current NMC PIN registration.
  • Minimum 2 years' post-qualification experience.
  • Minimum 5 years' experience within health and social care.
  • Experience within community-based complex care.
  • Experience managing and supervising nurses or care teams.
  • Strong knowledge of health and social care legislation and CQC requirements.
  • Excellent communication and relationship-building skills.
  • Highly organised with strong clinical leadership skills.
  • Full driving licence and access to your own vehicle.
  • Confident working with clients, families and multidisciplinary professionals
