Cityworx are recruiting on behalf of a client, a provider of personalised home care, who require an experienced Care Manager for their Amersham office.
The client are an established care business who require a Manager who has managed a good quality, customer focused, high end care service and has experience of private care services.
In this role, you will be responsible for
Responsibilities
- To undertake client assessments
- Accountable for client set up from assessment through to the client start date, including all associated paperwork
- To ensure compliance to all regulatory requirements through robust management of care being delivered.
- The set up and maintenance of care-related documentation, within agreed timescale.
- To ensure professional communication with all key professionals associated with the care delivery within placements
- To ensure robust completion of incident reporting
- To ensure all client and carer contact is captured as required on the system.
- To handle initial complaints/concerns & report actions to the Operations Manager
- To support the Operations Manager and Director with events and community functions.
- To support and participate in the on call function which will require some weekend working
