As Placement Monitoring Officer you can make a powerful contribution to the Adult Care team at Westminster Adult Social Services. You will manage a caseload of adults with complex needs, ensuring that their service plans are fit‑for‑purpose and that a good standard of care is being provided in residential placements, nursing homes and supported living facilities.

Responsibilities

  • Monitor the health and social care aspects of all Support Plans, ensuring a good standard of care is maintained.
  • Conduct holistic assessments, care checklists and reviews into finances.
  • Perform risk assessments that reflect an individual’s capacity to make decisions and acknowledge their right to take risks.
  • Undertake Safeguarding Adults Investigations and report outcomes in line with policy.
  • Prescribe equipment, assistive technology and minor adaptations to maximize independence.
  • Liaise with Social Services to ensure placements meet service specifications.
  • Work in partnership with families and carers, remaining sensitive to complex relationships.
  • Engage with other service providers and the voluntary sector to maximise community opportunities.
  • Support culturally diverse clients with a range of physical disabilities and mental health needs.

Qualifications and Experience

  • Extensive knowledge of legislation, statutory guidance and best practice in adult social work and health care.
  • Ability to translate laws and official guidance into clear, workable policies and day‑to‑day practice.
  • Experience leading multi‑agency teams to ensure effective service delivery.
  • Experience managing budgets and competing priorities in a complex, controlled environment.
  • Proven skill in constructing measures, metrics and performance indicators to shape and drive performance.
  • Highly experienced people manager capable of embedding organisational values and working diplomatically at a multidisciplinary level.
  • Commitment to Council policies relating to Equal Opportunity, Customer Care and service delivery.
  • Qualification in Social Work, Occupational Therapy or Nursing.
