As a Social Care Practitioner, you will contribute to our Joint Homelessness Team (JHT), delivering holistic, person-centred support to rough sleepers across Westminster, enabling independence and positive outcomes.
Responsibilities
- Adopt a flexible, creative, solution-focused approach to meet the varied needs of service users.
- Build trust and develop person-centred interventions for individuals from diverse and ethnic backgrounds.
- Perform statutory duties under the Care Act, Mental Health Act and Mental Capacity Act, including supporting users in making self‑assessments and arranging appropriate support.
- Identify urgent support requirements and work with users, carers and hospital professionals to determine and deliver timely assistance.
- Link users to community resources and provide clear, informative advice.
- Apply resource management skills to operate within finite finances and eligibility criteria.
- Use the Performance Assessment Framework for Social Services to guide service delivery.
- Organise workload, meet deadlines, set objectives and deliver on agreed outcomes.
- Communicate effectively with service users, carers, and other professionals, building productive relationships.
- Engage in continuous learning, undertake training and adapt to new methods and developments.
Qualifications & Requirements
- NVQ Level 3 or 4 (or equivalent) in Health and Social Care / Promoting Independence.
- Strong literacy skills and the ability to conceptualise information.
- Experience working with adults with physical or mental health problems and a clear understanding of the legal frameworks (Care Act, Mental Health Act, Mental Capacity Act).
- Respect for cultural diversity and competence in dealing with disadvantaged communities.
- Problem‑solving, resource management and performance‑framework application skills.
