As a Social Worker with our Hospital Discharge Team you can make your own powerful contribution to Westminster's success. Based in St Mary’s Hospital and operating across the Borough of Hammersmith and Fulham, the Royal Borough of Kensington and Chelsea and Westminster City Council, you'll work closely with health colleagues within an acute setting, and be an active member of a ward multi‑disciplinary team. You will undertake assessments of need and develop support plans that help patients with their discharge from hospital, undertake safeguarding investigations and Mental Capacity Act assessments, and work with carers and other professionals to minimise and manage risk while helping each patient to maintain their chosen lifestyle.

Key Responsibilities

  • Provide advice and information to colleagues and stakeholders on the role of the social worker and the services we provide.
  • Participate in the urgent response and duty system and contribute to the development of the service.
  • Ensure professional registration and DBS are kept up to date, and manage your own CPD.
  • Participate in appraisals, audits, evaluations, reflective practice and team meetings.
  • Work in line with all policies and procedures, including health and safety, data and case recording, supervision, data protection and information governance.
  • Deliver best interest and practice teacher training and Trusted Assessor training to prescribe simple equipment and assistive technology.
  • Adopt an inter‑disciplinary approach and collaborate with all partners to achieve best outcomes.

Working Pattern

This role involves working five days a week, including Saturdays, Sundays and Bank Holidays. You will work as part of a team three days a week, and be the only Social Worker on site over weekend and holiday periods, with a Duty Manager available remotely for support.

Qualifications & Experience

  • Qualification: CQSW/Diploma in Social Work, a Degree in Social Work or its equivalent, and registration as a member of Social Work England.
  • Post‑qualifying experience in a relevant Local Authority or Health setting.
  • Well‑developed knowledge of relevant legislation and a good understanding of personalisation, independence and choice.
  • Experience working with older or physically disabled people in a Social Services Duty System.
  • Experience of assessment work in a multi‑disciplinary acute setting.

Essential Skills

  • Excellent written and verbal communication skills and ability to produce high‑quality reports.
  • Strong case recording and assessment skills, with an understanding of budgets and financial controls.
  • Ability to complete robust risk assessments and prioritise workloads to meet deadlines.
  • Confidence in using Microsoft’s suite of software.
  • Ability to negotiate with other agencies on behalf of the Commissioning Authority and service users.

Desirable: ability to communicate professionally in a language other than English.
