City of York Council (CYC) Adult Social Care (ASC) are looking to recruit one Social Worker to join the Hospital Social Work Team (HSWT). This is a qualified position.

Main duties of the job:

  • The role would require you to work within a fast-paced environment, within a supportive Hospital Social Work Team facilitating timely and safe hospital discharges.
  • The team works to facilitate rapid hospital discharges and complete assessments post discharge in line with the Discharge to Assess Guidance.
  • Regular supervision and training provided.
  • You will hold your own caseload working within a supportive team of qualified and unqualified workers.

Job responsibilities:

To deliver a high quality, strengths based and accountable social care service, ensuring that adults (with the support of their carers/families/networks) can achieve good outcomes. The role works with people to enable them to:

  • Seek asset-based solutions within a strengths-based model
  • Assess, manage and review their own needs and risks
  • Maximise their independence and self-direct their responses to deliver agreed outcomes
  • Access personalised care and support to meet their assessed needs while maintaining themselves in the community
  • Understand risk and be safe
