Our ambition is to create a more responsive, strengths-based approach that maximises independence, prevents escalation of need, and delivers the best possible experience for those accessing adult social care. The redesigned service will bring together a multi-disciplinary team, including Social Workers, Occupational Therapists, Telecare Specialists and Local Area Coordinators, to review contacts collaboratively and resolve issues wherever possible at the first point of contact.
About The Role
This is a unique opportunity to shape the development of the service and bring the valuable skills and expertise of the Local Area Coordination model into the heart of adult social care.

- Expand and strengthen the team's knowledge of community assets, networks and resources.
- Develop stronger links and visibility within local communities.
- Support a proactive approach to building community resilience and reducing reliance on formal services.
- Contribute to the day-to-day operational management and decision-making processes within the team.
- Provide line management and support to the team.
- Work closely with colleagues across the multi-disciplinary team to promote strengths-based practice and innovative solutions.
What We're Looking For:
We are seeking an experienced and motivated Senior LAC who:

Why Apply?
This secondment offers a fantastic opportunity to influence the future direction of adult social care's Front Door service, contribute to an innovative and evolving model of practice, and further develop your leadership experience within a dynamic multi-disciplinary team. If you are passionate about prevention, community-based solutions and improving the experience of people accessing support, we would love to hear from you.
