The Transitions Social Worker role in York sits within the Preparation for Adulthood Service, bridging children’s and adult services to support young people with SEND into adulthood in a timely, person‑centred way.
You will work with Children’s Social Care, SEND, education and health partners to plan early and collaboratively, leading on assessments and complex caseload management while promoting independence and community inclusion.
