What the Job Involves
This frontline role is based at the Westminster Community Independence Service (CIS) Reablement Team, where you will support residents in regaining independence following hospital discharge or health crises. Working Monday to Friday with an option for flexible hours, you'll deliver skilled Occupational Therapy interventions, manage cases with goal-focused reablement plans, conduct holistic assessments, and develop and implement intervention plans, including complex assessments and equipment allocation. Collaboration with multidisciplinary teams, including Occupational Therapists, Independent Living Assessors, and Social Workers, is key to ensuring effective service delivery. You'll maintain accurate case records using the Mosaic system and receive ongoing professional development and supportive supervision.
