Working closely with health colleagues in the Joint Homelessness Team based in Vauxhall Bridge Road. Supervised by the senior social worker, the social worker will carry out local authority duties in relation to older adults with a combination of mental and physical health problems.

Responsibilities

  • Carry out Care Act assessments and reviews
  • Set up care packages and review the care packages
  • Hospital discharge planning and, where necessary, take cases to funding panels for appropriate intensive supportive care packages or placements
  • Carry out assessments under the Mental Capacity Act and fulfill safeguarding duties
  • Multidisciplinary working and joint visits with mental health colleagues
