This role helps older people live well at home by providing expert medical care, making a real difference to their independence and quality of life. You'll join a skilled, supportive multidisciplinary team including geriatricians, nurses, therapists and social care colleagues. Together, you'll deliver person-centred care across community settings, prevent unnecessary hospital admissions, and support people with complex frailty needs.

Main duties of the job
- Provide medical assessment, diagnosis and treatment for older people with frailty.
- Support urgent community response and virtual ward care.
- Lead on complex case management and develop clear care plans.
- Work closely with multidisciplinary teams, care homes and primary care.
- Contribute to service improvement, teaching and clinical governance.
Qualifications
What we're looking for:
- A qualified doctor with full General Medical Council (GMC) registration.
- Experience in geriatric medicine or caring for older people.
- Strong clinical judgement and decision‑making skills.
- Confidence working both independently and as part of a team.
- Good communication skills with patients, families and colleagues.
Essential Qualities
- MBBS or equivalent medical qualification.
- Full registration with the GMC.
Desirable Qualities
- Specialty Certificate in Geriatric Medicine.
- Experience of working in geriatric medicine.
- Understanding of audit and research.
- Evidence of active participation in audit, research, teaching and learning.
