This is a great opportunity to build your specialist skills while supporting people recovering from stroke to regain independence and confidence. You'll join a supportive, multidisciplinary stroke rehabilitation team working across the community, use your clinical expertise to assess needs, plan interventions, and help people work toward goals that matter to them. You'll also play a key role in shaping safe, effective, and personācentred care across the service.
Main duties of the job
- Deliver specialist assessments and interventions for people with complex strokeārelated needs
- Plan and manage your own caseload, making clear clinical decisions and prioritising care
- Offer advice, supervision and training to colleagues, students, patients and carers
- Work with others to triage referrals, monitor service quality and support service development
- Maintain accurate records and follow professional and organisational policies
Qualifications (Essential)
- Current HCPC professional registration
- Degree or diploma in Physiotherapy or Occupational Therapy
Qualifications (Desirable)
- Postāgraduate diploma in a relevant specialist area or a professional qualification to degree level supplemented by specialist accredited training and experience equivalent to a postāgraduate diploma level
Experience (Essential)
- Evidence of sound postāregistration professional practice
- Evidence of continuing professional development (CPD) relevant to the clinical area at a specialist level of care
- Experience of multiāagency/professional working
- Experience of case load management within specialist service area
- Evidence of promoting/supporting active patient/relative involvement/participation
Experience (Desirable)
- Experience of complex case load management within specialist service area
