The predicted salary is between 37338 - 44962 £ per year.
This role helps people with long-term respiratory conditions receive safe, effective care at home, making a real difference to their quality of life. You'll work as part of a supportive community respiratory team, alongside specialist nurses, GPs and other health professionals. Your work will support people who need home oxygen therapy, as well as those living with complex respiratory conditions, including palliative and end‑of‑life care. You'll see patients in their own homes and in clinics, helping them stay independent and well for longer.
What you'll do:
- Carry out holistic respiratory and home oxygen assessments in the community
- Manage a caseload and review care in line with national and local guidance
- Support people and carers with self‑management, education and treatment plans
- Work closely with multidisciplinary teams to plan and coordinate care
- Maintain clear, accurate records using electronic systems
Qualifications:
- A registered nurse with post‑registration experience in respiratory or long‑term condition care
- Confidence working independently and making clinical decisions
- Strong communication skills and a calm, empathetic approach
- Willingness to keep learning, with support through training and supervision
