A Clinical Pharmacist position is available within the CHCP Frailty Service, this is a Bank position to support continued service provision.
Within the frailty team, the clinical pharmacists work within a specialist multi-disciplinary team; this includes Consultant Geriatricians, frailty doctors, advanced nurse practitioners, pharmacy technicians, and therapists. The specialist team provides a comprehensive geriatric assessment (CGA), identifying a person's medical, functional and social needs to develop a patient-centred care plan. The pharmacist provides expert clinical prescribing advice, medicine optimisation and structured medication reviews. Pharmacists work within a number of settings including the Jean Bishop Integrated Care Centre and care homes.
Key Responsibilities- Provide expert clinical prescribing advice, medicine optimisation and structured medication reviews.
- Work autonomously within a frailty team, collaborating with team members to develop patient-centred care plans.
- Experience of performing structured medication reviews is preferable but not essential.
- Provide training as part of an individualised induction programme.
- Demonstrate excellent communication, organisational skills, a focus on patient-centred care, and an interest in frailty.
- Work autonomously and as a reliable team member.

- Must have independent means of transport to travel around the geographic area our services operate across and work flexibly to meet the needs of the organisation.
