Responsibilities

  • Provide clinical pharmacy support within the Frailty Service.
  • Collaborate with multidisciplinary teams—doctors, specialist nurses, pharmacy technicians and therapists—to deliver comprehensive geriatric assessments.

• Carry out assessments for patients in the following settings

  • Jean Bishop Integrated Care Centre
  • Care homes
  • Community bedded units
  • Hospital at Home and Frailty Virtual Ward including home visits
  • Participate in related projects and services as required.
  • Work primarily Monday to Friday, 8:30am–4:30pm (part‑time, flexible and secondment arrangements will be considered).
  • Maintain accurate and timely medication review records.
  • Support the Medicines Service team in Hull through professional assistance.
  • Travel across the service area using independent transport.
  • Adapt working hours to meet organisational needs.

Qualifications & Skills

  • Qualified pharmacist with current registration.
  • Patient‑centred approach and interest in frailty care.
  • Excellent communication and organisational skills.
  • Experience with structured medication reviews is desirable but not essential.
  • Capability to work flexibly in a multidisciplinary environment.
