Responsibilities
- Provide clinical pharmacy support within the Frailty Service.
- Collaborate with multidisciplinary teams—doctors, specialist nurses, pharmacy technicians and therapists—to deliver comprehensive geriatric assessments.
• Carry out assessments for patients in the following settings
- Jean Bishop Integrated Care Centre
- Care homes
- Community bedded units
- Hospital at Home and Frailty Virtual Ward including home visits
- Participate in related projects and services as required.
- Work primarily Monday to Friday, 8:30am–4:30pm (part‑time, flexible and secondment arrangements will be considered).
- Maintain accurate and timely medication review records.
- Support the Medicines Service team in Hull through professional assistance.
- Travel across the service area using independent transport.
- Adapt working hours to meet organisational needs.
Qualifications & Skills
- Qualified pharmacist with current registration.
- Patient‑centred approach and interest in frailty care.
- Excellent communication and organisational skills.
- Experience with structured medication reviews is desirable but not essential.
- Capability to work flexibly in a multidisciplinary environment.
