We are recruiting two Pharmacy Technicians to support teams across our Frailty Service. Working within a multidisciplinary team of clinical pharmacists, doctors, specialist nurses and therapists, our Pharmacy Technicians will support with medication reconciliation and review for patients in:o the Jean Bishop Integrated Care Centreo care homeso community bedded units (intermediate care services)o the Hospital at Home Frailty Virtual Ward, including home visitsVacancies are available across these services and projects. Standard working hours are Monday to Friday, 8.30 am to 4.30 pm., You will work closely with healthcare professionals across our frailty services to optimise medicines, help patients take their medicines as prescribed, and respond to pharmacy-related queries from the care team.

As part of the multidisciplinary team, you will support patients, prescribers, nurses and social care colleagues to promote the safe and effective use of medicines. We provide an individualised induction programme and ongoing support from an experienced frailty team. CHCP also offers development and training opportunities tailored to your learning needs. You will be patient-centred, interested in frailty, and an excellent communicator and organiser, with the confidence to engage and influence a wide range of healthcare professionals. Applicants must hold an NVQ Level 3 qualification in Pharmacy Services and be registered with the General Pharmaceutical Council.
