Clinical Pharmacist position within CHCP Frailty Service, a bank position to support continued service provision.
The frailty team is a specialist multi‑disciplinary team that includes Consultant Geriatricians, frailty doctors, advanced nurse practitioners, pharmacy technicians, and therapists. The pharmacist provides expert clinical prescribing advice, medicine optimisation and structured medication reviews as part of a comprehensive geriatric assessment (CGA) providing patient‑centric care plans. Clinical pharmacists in this post work in the Jean Bishop Integrated Care Centre and care homes.
Key Responsibilities- Provide expert clinical prescribing advice and medication optimisation.
- Perform structured medication reviews.
- Develop patient‑centred care plans within multidisciplinary consultations.
- Work autonomously and as a reliable team member.
Experience of performing structured medication reviews is preferable but not essential. Applicants from all pharmacy sectors are welcome. Candidates should demonstrate excellent communication and organisational skills, a focus on patient‑centred care, and an interest in frailty.
Support and TrainingTraining will be provided as part of an individualised induction programme. You will receive professional support from the Medicines Service team based in Hull. Independent transport is required to travel within the geographic area of our services and to work flexibly to meet organisational needs.
