We are seeking an enthusiastic and dynamic Band 4 Associate Practitioner to join our well‑established and supportive multidisciplinary Falls Service.

Responsibilities

  • You will play a key role in providing holistic assessment and interventions to patients who have experienced a fall, with the opportunity to visit patients in their own homes and care homes across the Hull and East Riding area.
  • In this role, you will manage your own caseload of patients, delivering falls prevention and rehabilitation interventions under the supervision of senior clinicians.
  • You will be responsible for conducting comprehensive assessments, developing individualized care plans, implementing tailored interventions to reduce the risk of further falls and educating patients and their families about falls prevention strategies.
  • The service operates on a 7‑day working pattern, and as part of the team, there is an expectation to contribute to this, including working on public holidays.

Requirements

  • Experience in fall assessment and rehabilitation with adults.
  • Ability to travel across the Hull and East Riding geographical area; a full UK driving licence and access to a vehicle are required.
  • Strong communication skills and ability to educate patients and families.
  • Commitment to work within a multidisciplinary team and deliver high‑quality care.
