Salary: ÂŁ25,364 + on call payments

Driving licence is required

Monday to Friday 9am - 5pm with additional on call duties on a rota basis

Job Description

What you’ll do:

  • Coordinate Care: Manage referrals, create care worker rotas, liaise with professionals (e.g., social workers, GPs), and monitor service users' conditions.
  • Staff Management: Supervise care workers, conduct appraisals.
