What you’ll do:
- Support the delivery of the highest quality care in the community
- Complete assessments and reviews of individual care services
- Provide practical supervision, instruction, and support to Care Assistants
- Work with Care Coordinators to plan effective and realistic care worker rotas
- Carry out regular needs and risk assessments, including renewed and repeated reviews
- Participate in multi-disciplinary reviews of service users’ needs
- Shadow new care workers to ensure their competence and confidence
- Observe and evaluate care worker practice as part of routine supervision or when performance concerns arise
- Provide personal care with dignity and respect
- Complete essential administrative tasks linked to care delivery and service quality
Qualifications:
You’ll need experience in:
- Carrying out initial risk and needs assessments for individual service users, either in their own homes or in other settings such as hospitals or residential/nursing homes
- Working in partnership with service users, their families, carers, advocates, healthcare professionals and other stakeholders to ensure seamless, holistic care
- Supporting high-quality care delivery and contributing to safe, person-centred care planning
- Communicating clearly, professionally, and with compassion when coordinating care and reviewing needs
Have a full UK driving licence and access to your own vehicle.
