APP Adviser – Alder Hey
Citizens Advice Liverpool has commissioned a dedicated welfare service to support patients and families vulnerable to the wider determinants of health. The service targets patients at risk of mental distress and exacerbations in existing health conditions, aiming to reduce health inequalities across Liverpool by supporting families who are struggling to make ends meet and lowering demands on health services associated with non‑clinical need.
Responsibilities
- Provide enhanced support to clients from first point of entry through the service.
- Move between frontline advice and follow‑up appointments for multiple issues to support rapid response time to the most vulnerable clients.
- Support a better client journey overall by offering a comprehensive package of information, advice and support in relation to welfare benefits, debt management, employment and housing.
- Work closely with colleagues from CAL’s Health Programme and collaborate with Link Workers across Liverpool’s Primary Care Networks.
