Chelsfield Park Hospital in Kent is part of Circle Health Group, Britain\’s leading provider of independent healthcare with a nationwide network of hospitals & clinics, performing more complex surgery than any other private healthcare provider in the country.
We have an opportunity for a Clinical Pharmacist to join their team of staff in the Pharmacy Department.
This is a Part time role for 21 hours a week. The role holder will be required to cover a flexible shift pattern.
Duties Of This Role Include
- Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
- Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
- Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
- Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
- Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
- Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity
- Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance
Applicants Should Meet The Following Criteria
- A degree in pharmacy MPharm, BSc or BPharm.
- GPhC registration as a Pharmacist.
- Previous experience in a clinical hospital pharmacy role.
- At least 2 years of post qualification experience
- Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
- Excellent communication and clinical decision-making skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Confident using electronic prescribing and medication administration systems (e.g., EPMA).
