Responsibilities
- Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
- Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
- Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
- Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
- Promote safe and cost-effective use of medicines by supporting adherence to local and national policies.
- Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.
- Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.
Qualifications
- Degree in Pharmacy (MPharm, BSc or BPharm).
- GPhC registration as a Pharmacist.
- Previous experience in a clinical hospital pharmacy role.
- Strong knowledge of therapeutics, clinical guidelines and pharmaceutical legislation.
- Excellent communication and clinical decision‑making skills.
- Ability to work autonomously and as part of a multidisciplinary team.
- Confidence using electronic prescribing and medication administration systems (e.g. EPMA).
