Clinical Pharmacist

Chelsfield Park Hospital, part of Circle Health Group, located in Kent.

Position: Part‑time, 21 hours per week with a flexible shift pattern.

Duties

  • Conduct medication reviews for inpatients and outpatients, identifying interactions, contraindications, and opportunities for medicines optimisation.
  • Provide tailored advice to patients on their treatment plans, including side effects, compliance, and lifestyle considerations.
  • Support safe prescribing through reconciliation, clinical screening, and dose adjustment.
  • Review and authorise prescriptions in line with clinical guidelines, formulary restrictions, and regulatory requirements.
  • Promote safe and cost‑effective use of medicines by supporting adherence to local and national policies.
  • Collaborate with procurement, stock control, and pharmacy technicians to ensure medicine availability and supply continuity.
  • Monitor and report medication incidents, near misses, and adverse drug reactions in line with hospital protocols and MHRA guidance.

Qualifications

  • A degree in pharmacy (MPharm, BSc, or BPharm).
  • GPhC registration as a Pharmacist.
  • Previous experience in a clinical hospital pharmacy role.
  • At least 2 years of post‑qualification experience.
  • Strong knowledge of therapeutics, clinical guidelines, and pharmaceutical legislation.
  • Excellent communication and clinical decision‑making skills.
  • Ability to work autonomously and as part of a multidisciplinary team.
  • Confidence using electronic prescribing and medication administration systems (e.g., EPMA).

  • Salary: Up to £51,000 per year (FTE basis, depending on level of experience, training and qualification).
