Calderdale Inclusion Support Service provides Mental Health Support in collaboration with South West Yorkshire NHS Trust. Within this service we are recruiting a Housing Support Worker to assist Calderdale adults with mental health needs to access and maintain suitable housing. Our objective is to offer timely advice and support to enable individuals to acquire, set up and retain secure tenancies.
Key Responsibilities
- Assess and identify housing needs, and develop and implement housing support plans in consultation with the individual, keeping them at the centre of the process.
- Plan for and arrange meetings and ensure case records are accurate and up to date; use IT packages and systems independently.
- Support individuals through the stressful event of moving home, understand coping mechanisms, and liaise with the wider support team to promote mental well-being.
- Work in a hybrid model from Council and NHS offices, with occasional work from home; undertake visits to local hospitals, residential schemes, clinics, and clients' homes; work independently to meet the needs of the service.
Key Requirements & Qualifications
- Full UK driving licence and access to a car.
- Experience working with adults who have mental health needs and in multi-agency settings, with knowledge of housing.
- Strong person-centred value base, empathy, effective interpersonal skills, initiative, problem-solving and ability to work well as part of a team.
- Level 3 Health and Social Care or equivalent qualification, or demonstrable experience.
