Role Overview
This is a new post created to assist with the Hospital Discharge process. Based within Central Operations in the Civic Centre, Motherwell, the role will involve liaising with Adult Health & Social Care and the NHS to improve discharge timescales. You will work with Housing Locality staff and Registered Social Landlords to secure sustainable housing solutions for patients who are unable to return to their homes due to medical issues.
Responsibilities
- Liaise with Adult Health & Social Care and NHS partners to improve patient discharge timelines.
- Collaborate with Housing Locality staff and Registered Social Landlords to identify and secure suitable housing for discharged patients.
- Collect and analyse data on discharge and housing outcomes, preparing reports for internal use.
- Maintain up‑to‑date knowledge of housing legislation and NHS discharge policy.
- Participate in the Out‑of‑Hours Homeless Standby Service roster during evenings and weekends.
Qualifications & Experience
- Education: SVQ Level 3 / HNC (SCQF Level 7) or equivalent in a relevant discipline.
- Experience in a housing or customer service environment; knowledge of housing legislation is desirable.
- Strong IT, verbal and written communication skills, including report writing.
- Committed to customer care and able to support vulnerable members of the community.
- Possession of a current driving licence.
- Registered or eligible for registration with the Scottish Social Services Council (SSSC).
- Must become a Protecting Vulnerable Groups (PVG) Scheme member prior to commencing.
- Practical experience across a front‑line housing service or similar front‑line fieldwork service is required for progression to the NLC9 grade.
Other Requirements
Temporary position until 15/07/2027. 35 hours per week – Monday to Thursday 08:45 – 16:45 (55 minute unpaid break), Friday 08:45 – 16:15 (50 minute unpaid break). Pool cars available for use by standby officers. Payments for standby periods will be made in accordance with the SJC Salaries Agreement.
