POSITION SUMMARY: The Resident Care Coordinator (RCC) supervises and evaluates the care of residents in a memory care and assisted living facility. Schedules and trains care staff, and ensures the clinical services are delivered in accordance with regulations and professional standards.
ESSENTIAL JOB FUNCTIONS:
- Supports the mission, vision, and goals of the organization, upholding and promoting company culture and vision.
- Coordinating patient care plans by liaising with all necessary care providers and medical professions to ensure patient needs are met.
- Coordinate follow-up appointments and refer patients to specialists as needed.
- Looks for ways to improve the quality of patient care provided by the facility.
- Works to create efficient systems and workflows within the clinic.
- Attend departmental meetings and stay up to date on all business initiatives related to the clinic.
- Meet with patients and patient families to ensure their needs are being met and resolve any issues or complaints that arise.
- Attends training courses at community and maintains up to date monthly, quarterly, annual training through designated Learning Management System (LMS).
- Attend all required all staff meetings.
- Other duties as assigned, and which relate to the success of the community and the care, comfort, and happiness of our residents.
Requirements:
- EXPERIENCE, QUALIFICATIONS may require frequently manipulating weights of 25 pounds or more.
- Squat – must be able to squat on a frequent basis to reach below the knee, may require lifting of 50 pounds or less.
- Twist or rotate – must be able to twist or rotate occasionally at the trunk, hips, knees, and neck, manipulating weights of 25 pounds or more.
