HCRG Care Group in Gravesend is seeking a Band 3 Rehabilitation Assistant to join the Early Supported Discharge team. You will support patients recovering from stroke and neurological conditions, delivering prescribed rehabilitation activities in patients' homes, community settings and inpatient services, under therapists and nurses. You will work independently and as part of a multidisciplinary team, promoting dignity and person-centred care, with a driving licence for travel to patients.
