Everyday people with chest, heart and stroke conditions are leaving hospital scared and alone. You can be part of our mission to make sure that there is no life half lived in Scotland. By joining Chest Heart and Stroke Scotland (CHSS) as a Supported Self Management Services Manager you can be the difference between people just surviving and really living.

As Supported Self-Management Services Manager, you will lead the operational delivery and ongoing development of CHSS’s flagship self management programmes, including the Chronic Disease Self Management Programme and the Living Well with Aphasia programme. These nationally recognised programmes empower people to build skills, confidence and independence, supporting them to live well with and beyond their condition. Your leadership will ensure high quality, accessible and resilient delivery across Scotland, responding to local needs while maintaining national consistency.

You will be responsible for:

  • Leading and supporting teams to deliver evidence based self management programmes
  • Ensuring continuity and resilience of services across your designated area
  • Implementing local operational plans to increase reach, manage risk and safeguard delivery

A key aspect of this role is embedding a culture of continuous improvement, quality assurance and learning. You will monitor performance, use data and lived experience feedback to identify opportunities for improvement, and ensure services consistently meet CHSS standards, regulatory requirements and community needs. You will champion evidence-based practice and support innovation that strengthens outcomes for people affected by long term conditions and communication disability.

In addition, there will be the requirement of the role holder to be a visible and vocal advocate for the charity that ensures our position as a key 3rd sector partner within NHS Health Board areas and other geographic systems of health and social care. The active and continuous development of senior system relationships will underpin the delivery of the role and ensure the long term success of the charity and the delivery of our No Life Half Lived Strategy.

The postholder will initially cover service delivery across Grampian; this may expand into other health boards in the North as we continue to increase service provision. In addition, travel across Scotland will be required for training.
