What you’ll do
- Provide one-to-one support to residents, including home visits and advocacy
- Help residents navigate health, social care, and community services
- Co-produce personalised wellbeing and tenancy plans

About you
You’re a natural connector with resilience, empathy, and organisational skills. You can work independently and as part of a team, comfortable visiting residents in their homes.
We’re looking for the following skills and experience
- Experience supporting vulnerable people or those facing social exclusion
- Strong communication and relationship-building skills
- Ability to assess needs and manage risks appropriately
- Knowledge of safeguarding good practice
- Basic IT skills (Word, Excel, email)
- A Level 3 qualification (or equivalent experience) in a relevant field
- A full UK driving licence and access to a car
